Sports Packet

Every student athlete must have a completed sports packet on file before they can take part in a sport, whether it be tryouts, practice or competition.
 
 
The sports packet includes a physical that must be done by a licensed physician. Physicals done by chiropractors are not acceptable. Look below for the sports physical form that you must fill out and take with you to your physical appointment.
 
NOTEThe first page of the physical form, the page you fill out, stays at the doctor's office. The second page is the form that must be filled out and signed by the physician and returned to school with the rest of the sports packet. It is critical that you turn in the second page of the physical form.
 
Please plan ahead and get your completed sports packets turned in well in advance of when your sports season starts.


IMPORTANT

You MUST turn in these FIVE completed forms before your child can take part in sports:


  •  1. Athletics Emergency Card
  •  2. Athletics Participation Agreement
  •  3. Sports Physical Form (signed by physician, see Note above)
  •  4. Sports Safety Awareness
  •  5. Tri-Valley Athletic League Participation Contract

Volunteer clearance and driver clearance forms must also be completed and turned in if you plan to help transport athletes to events. The driver clearance form must be accompanied by a current copy of your vehicle's insurance coverage.  Volunteer Clearance is processed online here.
